My Testosterone Is Normal—So Why Do I Still Have Symptoms?
A normal testosterone result doesn't always mean hormones aren't part of the picture. Learn why free testosterone, SHBG, and your full clinical picture matter.

You’ve been tired. Your energy isn’t what it used to be. Your libido has declined. Perhaps you’re having more difficulty maintaining muscle, your workouts don’t produce the same results, your body composition has changed, or you simply don’t feel like yourself.
So you have your testosterone checked.
The result comes back:
“Normal.”
Does that mean hormones cannot be contributing to your symptoms?
Not necessarily.
A laboratory reference range is an important clinical tool, but a testosterone result should be interpreted in the context of the individual patient. Total testosterone is only one part of a much larger picture that can include symptoms, free testosterone, sex hormone-binding globulin (SHBG), estradiol, other hormones, medications, sleep, metabolic health and underlying medical conditions.
At R2 Medical Clinic, we approach men’s hormone health differently from simply asking whether a testosterone result has an “H” or “L” next to it.
We treat the patient, not simply the laboratory number.
At the same time, symptoms alone do not prove that a man has testosterone deficiency—or that testosterone therapy is the answer.
The first question should be:
Why am I experiencing these symptoms?
What Does a “Normal” Testosterone Level Mean?
Laboratories establish reference ranges based on values observed within a reference population.
If your result falls within that range, the laboratory generally reports it as normal.
But a reference range should not be confused with a universally “optimal” testosterone level for every man.
Men are biologically different.
Two men can have identical total testosterone levels yet have very different clinical presentations.
One may have excellent energy, normal sexual function, good muscle mass and no hormone-related complaints.
Another man with the same total testosterone result may experience reduced libido, declining energy or other symptoms.
That doesn’t prove the second man’s testosterone is causing his symptoms.
But it does mean that the clinical evaluation shouldn’t necessarily end with the word “normal.”
Testosterone Deficiency and Hormone Optimization Are Not the Same Thing
This distinction is important.
Having symptoms with a “normal” testosterone level does not automatically mean you need testosterone.
It means you may deserve a more comprehensive evaluation.
Major medical guidelines define male hypogonadism or testosterone deficiency using both clinical and laboratory criteria.
The American Urological Association considers a total testosterone level below approximately 300 ng/dL a reasonable cutoff supporting the diagnosis when appropriate symptoms or signs are present and low levels are confirmed.
The Endocrine Society similarly recommends diagnosing hypogonadism in men who have compatible symptoms or signs together with unequivocally and consistently low testosterone concentrations.
Hormone optimization is a broader clinical approach.
It begins by asking whether a patient’s symptoms could have a hormonal component and examining the complete clinical picture rather than deciding that everything is fine because one laboratory result falls within a population reference range.
Total Testosterone Doesn’t Always Tell the Whole Story
Most testosterone circulating in the bloodstream is bound to proteins.
One of the most important of these proteins is sex hormone-binding globulin, or SHBG.
Only a small proportion of circulating testosterone exists as free testosterone.
Because SHBG levels differ among individuals, two men with the same total testosterone level may have different free testosterone concentrations.
This becomes particularly important when total testosterone doesn’t seem to correspond with the clinical picture.
What Is Free Testosterone?
Total testosterone represents testosterone circulating in the bloodstream in both protein-bound and unbound forms.
Free testosterone refers to the small fraction that is not bound to circulating proteins.
Current Endocrine Society guidance recognizes situations in which free testosterone can provide useful additional diagnostic information—particularly when total testosterone is near the lower end of the reference range or when a condition affecting SHBG is present.
This can sometimes explain an apparent contradiction:
The total testosterone looks acceptable, but the free testosterone may tell a different story.
Free testosterone measurements also require careful interpretation because not all laboratory methods are equally reliable.
What Is SHBG—and Why Does It Matter?
SHBG is produced primarily by the liver and binds testosterone and other sex hormones in the bloodstream.
If SHBG is relatively high, a man’s total testosterone can appear reasonably good while his free testosterone may be lower.
Conversely, low SHBG can produce a relatively low total testosterone while free testosterone remains adequate.
SHBG can be influenced by multiple factors, including:
- Age
- Obesity
- Insulin resistance
- Thyroid function
- Liver disease
- Certain medications
- Other hormonal conditions
This is one reason looking only at total testosterone can sometimes provide an incomplete picture.
What Symptoms May Be Associated With Testosterone Deficiency?
Symptoms associated with testosterone deficiency can include:
- Reduced libido
- Loss of spontaneous or morning erections
- Erectile dysfunction
- Reduced energy
- Reduced muscle mass
- Decreased strength or exercise performance
- Increased body fat
- Reduced motivation
- Difficulty concentrating
- Mood changes
- Fertility problems
- Reduced bone density
But there is an important problem:
Most of these symptoms aren’t unique to low testosterone.
Fatigue doesn’t automatically mean low testosterone.
Erectile dysfunction doesn’t automatically mean low testosterone.
Difficulty building muscle doesn’t automatically mean low testosterone.
That’s why a good hormone evaluation needs to look beyond testosterone as well as beyond the laboratory reference range.
If Testosterone Is Normal, What Else Could Be Causing My Symptoms?
There are numerous possibilities.
Sleep
Poor sleep can have profound effects on energy, mood, cognition, sexual function, metabolism and overall well-being.
Sleep apnea is particularly important in men and can sometimes go undiagnosed.
Thyroid Function
Both underactive and overactive thyroid function can produce symptoms that overlap with hormone-related complaints.
Obesity and Metabolic Health
Obesity, insulin resistance and metabolic disease can affect testosterone physiology while independently contributing to fatigue, sexual dysfunction and changes in body composition.
Medications
Certain medications can affect testosterone production, sexual function, energy or other symptoms.
These can include opioids, corticosteroids and various other medications depending on the patient.
Cardiovascular Health
Erectile dysfunction can sometimes be a vascular issue rather than primarily a testosterone issue.
Cardiovascular risk factors therefore matter when evaluating sexual symptoms.
Psychological Factors
Stress, depression and anxiety can affect energy, concentration, sleep, libido and sexual performance.
Recognizing these possibilities doesn’t mean symptoms are “all in your head.” It means these symptoms can have multiple biological and psychological contributors.
Other Hormonal Abnormalities
Depending on the patient’s history and symptoms, thyroid hormones, prolactin, estradiol, LH, FSH and other laboratory findings may provide useful information.
Could Estradiol Be Part of the Picture?
Possibly.
Men need estradiol.
A portion of testosterone is naturally converted into estradiol through the aromatase enzyme.
Estradiol contributes to several aspects of male physiology, including bone and sexual health.
Both the testosterone-to-estradiol relationship and an individual’s symptoms can be relevant when evaluating hormone health.
An estradiol result should not automatically be treated simply because it falls above or below a particular laboratory value.
Likewise, suppressing estradiol unnecessarily can create problems of its own.
The entire hormonal picture matters.
Does Age Matter?
Yes—but age alone shouldn’t explain away every symptom.
Testosterone physiology changes with aging, and SHBG often changes as men get older.
At the same time, fatigue, loss of strength, declining sexual function and changes in body composition may have numerous causes that become more common with age.
A man shouldn’t automatically assume:
“I’m getting older, so this is just how I’m supposed to feel.”
But aging also isn’t, by itself, a diagnosis requiring testosterone treatment.
The appropriate approach is to determine what may actually be contributing to the symptoms.
Should Testosterone Be Rechecked?
Sometimes.
Testosterone levels vary from day to day and can be influenced by timing, illness, nutrition and other factors.
For the formal diagnosis of testosterone deficiency, major guidelines recommend confirming low testosterone with repeat early-morning testing.
If a testosterone result doesn’t seem consistent with the clinical picture, a clinician may determine that repeating the test or obtaining additional laboratory information is appropriate.
One isolated testosterone measurement should rarely be viewed as the complete assessment of a man’s hormonal health.
What Does a More Complete Hormone Evaluation Look Like?
Testing should be individualized rather than ordering every available hormone on every patient.
Depending on symptoms, medical history and previous results, evaluation may include:
- Total testosterone
- Free testosterone when appropriate
- SHBG
- Estradiol
- LH
- FSH
- Prolactin
- Complete blood count
- Thyroid testing
- Metabolic testing
- PSA when clinically appropriate
- Other laboratory studies based on the patient’s medical history
The purpose isn’t to find an abnormal number that can be treated.
The purpose is to understand the patient.

If My Testosterone Is Normal, Can It Still Be Optimized?
This requires an individualized medical discussion.
At R2 Medical Clinic, hormone optimization doesn’t mean attempting to push every man’s testosterone to the top of—or beyond—a reference range.
There is no scientifically established testosterone number that represents the perfect level for every man.
Instead, we evaluate the relationship between the patient’s symptoms, hormone levels, overall health and potential alternative explanations.
When treatment is considered, the question isn’t simply:
“Can we make the testosterone number higher?”
The more meaningful questions are:
- Are these symptoms likely to have a hormonal component?
- Have other potential causes been considered?
- Is treatment medically appropriate for this individual?
- Does treatment produce meaningful improvement?
- Can treatment be provided while maintaining appropriate safety parameters?
Those are very different questions.
What If Treatment Raises My Testosterone but I Don’t Feel Better?
This is an extremely important point.
A higher testosterone level does not necessarily equal successful treatment.
If testosterone levels increase but the symptoms that prompted treatment don’t meaningfully improve, the diagnosis and treatment strategy should be reconsidered.
Simply increasing the dose further isn’t necessarily the answer.
The original symptoms may have another cause.
Successful hormone optimization should therefore consider both objective laboratory information and the patient’s clinical response.
More Testosterone Is Not Always Better
Hormone optimization should never be confused with testosterone maximization.
Higher testosterone exposure can increase the likelihood of adverse effects without necessarily providing additional benefit.
Potential issues associated with testosterone treatment include:
- Increased hematocrit
- Increased blood pressure
- Acne
- Changes in estradiol
- Suppression of natural testosterone production
- Reduced sperm production and fertility
- Other treatment-related adverse effects
The objective should be meaningful clinical improvement with appropriate medical monitoring—not achieving the highest possible testosterone number.
What About Fertility?
This discussion should occur before testosterone therapy begins.
Exogenous testosterone suppresses the hormonal signals responsible for normal testicular testosterone production and sperm production.
As a result, testosterone therapy can significantly reduce sperm counts and may cause infertility.
Men who want children now or may want children in the future should discuss their reproductive goals with their physician before beginning testosterone therapy.
Other approaches may be appropriate depending on the individual patient.
Frequently Asked Questions
Can testosterone be normal and free testosterone be low?
Yes. Differences in SHBG can affect the relationship between total and free testosterone. In selected men—particularly those with borderline total testosterone or conditions affecting SHBG—free testosterone may provide useful additional information.
Is 300 ng/dL an optimal testosterone level?
No. Approximately 300 ng/dL is used by the American Urological Association as a reasonable diagnostic cutoff supporting testosterone deficiency. It is not an “optimal” testosterone target for every man.
If my testosterone is 400 or 500, can I still have symptoms?
Yes, you can have symptoms. The more important question is what is causing them. Symptoms alone don’t establish testosterone deficiency, and a total testosterone of 400 or 500 ng/dL does not automatically mean testosterone therapy is appropriate. The complete clinical picture should be evaluated.
Can SHBG make my total testosterone misleading?
It can affect interpretation. Higher SHBG can result in a relatively higher total testosterone despite lower free testosterone, while lower SHBG can contribute to lower total testosterone despite adequate free testosterone.
Should every man have free testosterone tested?
Not necessarily. Free testosterone can be particularly useful when total testosterone is borderline or when SHBG abnormalities are suspected.
Can low testosterone cause erectile dysfunction?
Testosterone deficiency can contribute to sexual dysfunction, but ED has many possible causes, including vascular disease, medications, neurologic conditions and psychological factors.
Can testosterone help with fatigue?
It may improve certain symptoms in appropriately selected men with testosterone deficiency, but fatigue is highly nonspecific. Sleep disorders, thyroid disease, anemia, medications, metabolic disease and many other conditions should be considered.
Do I need an estrogen blocker if I’m taking testosterone?
Not automatically. Estradiol has important physiologic functions in men. Treatment should be based on the individual clinical situation rather than routinely suppressing estradiol.
How do I know if hormone optimization is working?
Success shouldn’t be defined by the testosterone result alone.
Treatment should produce meaningful improvement in the symptoms being treated while laboratory and other safety parameters remain appropriate.

Don’t Treat a Number—Understand the Patient
Hormone medicine requires laboratory testing.
But laboratory testing is a tool, not the patient.
A “normal” testosterone result should not automatically end the investigation when a man continues to experience significant symptoms.
Likewise, symptoms should not automatically lead to testosterone treatment.
The better approach lies between those extremes:
Listen to the symptoms.
Evaluate the complete hormonal picture.
Look for other potential causes.
Individualize treatment when appropriate.
Measure whether the patient actually improves.
Monitor treatment for safety.
