Testosterone & Hormone Optimization for Men: A Physician’s Guide
A physician's guide to evaluating and treating low testosterone in men — symptoms, causes, lab evaluation, and individualized treatment options.

When a man experiences declining energy, reduced libido, changes in sexual function, difficulty maintaining muscle, changes in body composition, decreased motivation or other symptoms commonly associated with hormonal changes, the answer is not always as simple as determining whether his testosterone level falls inside or outside a laboratory’s reference range.
Laboratory values are important—but they are only one part of the clinical picture.
At R2 Medical Clinic, our approach to men’s hormone health is based on individualized hormone evaluation and optimization. We consider symptoms, laboratory findings, medical history, age, medications, lifestyle, metabolic health and treatment goals rather than treating a laboratory number in isolation.
A man can have a testosterone result that falls within a laboratory’s reference range and still experience symptoms that warrant further evaluation. That does not automatically mean testosterone therapy is appropriate. It means the evaluation should go deeper.
The objective of hormone optimization is not simply to produce a higher testosterone number. It is to identify potential contributors to a man’s symptoms and, when treatment is appropriate, determine an individualized approach that improves symptoms while maintaining appropriate medical monitoring and safety.
What Is Testosterone?
Testosterone is the primary androgen in men and plays an important role throughout adult life.
Testosterone contributes to:
- Sexual desire and function
- Muscle mass and strength
- Bone health
- Red blood cell production
- Body composition
- Sperm production and fertility
- Energy and general well-being
Testosterone production is regulated through a complex hormonal system involving the hypothalamus, pituitary gland and testes.
Levels also vary considerably among individuals.
What Does a “Normal” Testosterone Level Actually Mean?
This is one of the most important concepts in understanding men’s hormone health.
A laboratory reference range describes the range of values observed within a reference population. It does not necessarily identify the testosterone level at which every individual man will feel or function his best.
A result marked “normal” therefore needs clinical context.
Consider two men with the same total testosterone level. One may have excellent energy, libido, sexual function and body composition and have no reason to consider hormone treatment.
The other may be experiencing significant symptoms.
The laboratory result is identical. The clinical presentations are not.
That does not establish that testosterone is responsible for the second man’s symptoms. It does, however, justify looking beyond a simple “normal” flag on a laboratory report.
Testosterone Deficiency vs. Hormone Optimization
It is important to distinguish these concepts.
Major professional guidelines generally define testosterone deficiency or male hypogonadism as the combination of compatible symptoms or signs and consistently low testosterone concentrations.
The American Urological Association uses a total testosterone level below approximately 300 ng/dL as a reasonable laboratory cutoff supporting the diagnosis, together with symptoms and repeated morning testing.
Hormone optimization is a broader clinical approach.
Rather than asking only:
“Is this testosterone result below the laboratory range?”
we also ask:
“What symptoms is this patient experiencing, what could be causing them, and what does his complete hormonal and medical picture show?”
A man whose testosterone is within the reference range does not automatically have testosterone deficiency, and symptoms alone do not establish hypogonadism.
However, persistent symptoms deserve an appropriate evaluation rather than being dismissed simply because a total testosterone result was labeled “normal.”
Symptoms That May Warrant a Hormonal Evaluation
Symptoms that can occur with testosterone deficiency or other hormonal abnormalities include:
- Reduced libido
- Erectile or sexual dysfunction
- Loss of spontaneous or morning erections
- Reduced energy
- Difficulty maintaining muscle
- Reduced strength, exercise performance, slow recovery
- Increased body fat
- Changes in motivation
- Difficulty concentrating
- Changes in mood
- Reduced sense of well-being
- Decreased bone density
- Fertility problems
Importantly, these symptoms are not specific to testosterone.
Similar symptoms can result from thyroid disease, anemia, sleep disorders, obesity, metabolic disease, depression, medications, cardiovascular disease, nutritional problems and numerous other conditions.
A thoughtful hormone evaluation therefore attempts to determine the cause rather than assuming every symptom is caused by testosterone.
Why Total Testosterone Doesn’t Tell the Entire Story
Total testosterone is an important measurement, but it may not provide the complete picture for every patient.
Most circulating testosterone is bound to proteins, particularly sex hormone-binding globulin (SHBG) and albumin.
Only a relatively small portion circulates as free testosterone.
Differences in SHBG can therefore affect the relationship between total testosterone and the amount of testosterone available to tissues.
In selected patients, evaluating free testosterone and SHBG may provide useful additional information, particularly when total testosterone and the clinical presentation do not appear to agree.
This is one reason hormone evaluation should not be reduced to a single laboratory value.
Why Testosterone Levels Vary
Testosterone is not a static number.
Levels can vary because of:
- Time of day
- Sleep
- Acute illness
- Caloric restriction
- Certain medications
- Obesity
- Metabolic disease
- Chronic medical conditions
- Laboratory methodology
- Natural biological variation
When diagnosing traditional testosterone deficiency, current guidelines generally recommend confirming low testosterone with at least two appropriately obtained early-morning measurements.
When results and symptoms appear inconsistent, additional evaluation may be appropriate.
What Causes Testosterone Levels to Decline?
Testosterone can decline for numerous reasons.
Potential contributors include:
- Aging
- Obesity
- Insulin resistance and metabolic disease
- Sleep disorders
- Pituitary or hypothalamic dysfunction
- Testicular disorders
- Certain medications
- Opioid use
- Corticosteroid use
- Previous anabolic steroid exposure
- Chronic systemic illness
- Previous chemotherapy or radiation
- Certain genetic disorders
Identifying the underlying cause matters.
In some men, addressing weight, sleep, medications or another medical condition may improve testosterone levels and symptoms without testosterone therapy.
What Does a Comprehensive Men’s Hormone Evaluation Include?
There is no single laboratory panel appropriate for every man.
Evaluation begins with symptoms and medical history.
Depending on the individual patient, laboratory evaluation may include:
- Total testosterone
- Free testosterone
- SHBG
- Estradiol
- LH
- FSH
- Prolactin
- Complete blood count and hematocrit
- PSA
- Thyroid testing
- Metabolic testing
- Complete Blood Count (CBC)
- Complete Metabolic Panel (CMP)
- Additional testing based on symptoms and medical history
The purpose of laboratory testing should be to answer clinical questions—not simply to generate as many hormone values as possible.
What Is Hormone Optimization?
Hormone optimization is an individualized approach to evaluating and managing hormone-related symptoms.
It does not mean attempting to achieve the highest possible testosterone level.
More is not necessarily better.
Instead, the objective is to determine whether hormonal factors may be contributing to symptoms and, when treatment is appropriate, establish a level and treatment regimen that produces meaningful clinical benefit without unnecessary or excessive hormone exposure.

Symptoms + laboratory findings + medical history + risk factors + clinical response + ongoing monitoring.
The patient—not simply the laboratory report—is the focus of treatment.
Is There an “Optimal” Testosterone Number?
There is no scientifically established testosterone number that represents the ideal level for every man.
Men differ biologically.
A testosterone concentration associated with good sexual function, energy and well-being in one man may not correspond to the same clinical experience in another.
For that reason, we do not believe treatment should revolve around chasing a predetermined number.
Laboratory values provide important objective information and safety parameters, but clinical response matters as well.
The objective is an appropriate balance between symptom improvement and medical safety.
What If My Testosterone Is “Normal” but I Still Have Symptoms?
This situation deserves careful evaluation.
A testosterone result within a laboratory reference range does not prove that a man’s symptoms are unrelated to his hormones—but neither does it prove that testosterone therapy will help.
Several questions should be considered:
- Was testosterone measured appropriately?
- Was the result repeated when appropriate?
- What is the free testosterone?
- Is SHBG affecting interpretation?
- Are other hormones abnormal?
- Could medications be contributing?
- Could sleep, metabolic disease, thyroid dysfunction or another medical condition explain the symptoms?
- Are the symptoms actually consistent with androgen deficiency?
The appropriate response to a “normal” testosterone result in a symptomatic man is therefore not automatically “nothing is wrong” or “you need testosterone.”

Testosterone Treatment Options
When testosterone therapy is determined to be appropriate after individualized evaluation and informed discussion, several treatment options are available.
Testosterone Injections
Injectable testosterone allows individualized dosing and relatively straightforward adjustment based on symptoms, laboratory findings and clinical response.
Formulation and dosing frequency influence fluctuations in testosterone levels.
Topical Testosterone
Topical testosterone provides another adjustable treatment option and avoids injections.
Absorption can vary between patients, and precautions are necessary with some products to prevent unintended transfer through skin contact.
Testosterone Pellets
Testosterone pellets are implanted beneath the skin and gradually release testosterone over an extended period.
They can be convenient because treatment is infrequent, although the dose cannot be rapidly adjusted after implantation.
Other Testosterone Formulations
Additional FDA-approved formulations are available, including certain oral testosterone preparations.
No single delivery method is best for every patient.
Treatment should be selected according to the patient’s medical circumstances, preferences, response, convenience and treatment goals.
Hormone Optimization Is More Than Testosterone
Men’s hormone health involves more than total testosterone.
Depending on the patient, factors such as estradiol, SHBG, thyroid function, metabolic health and other hormonal systems may influence symptoms and treatment decisions.
This is why simply increasing testosterone without understanding the broader clinical picture may not produce the desired result.
Testosterone and Estradiol
Estradiol is not exclusively a female hormone.
Men naturally produce estradiol, primarily through conversion of testosterone by the aromatase enzyme.
Estradiol contributes to important physiologic functions in men, including bone and sexual health.
During testosterone therapy, estradiol may increase in some patients.
An elevated estradiol result does not automatically require treatment. Conversely, excessive suppression of estradiol can produce undesirable effects.
Estradiol should therefore be interpreted in the context of symptoms, testosterone levels, body composition and the overall clinical picture.
Testosterone and Fertility
Men who want children need to understand an important consequence of testosterone therapy.
Exogenous testosterone can suppress the hormonal signals from the pituitary gland that stimulate the testes. This can substantially decrease sperm production and may cause infertility.
Men who currently want children or may want children in the future should discuss fertility before beginning testosterone therapy.
Other treatment strategies may be appropriate depending on the individual circumstances.
Testosterone, Hematocrit and Red Blood Cells
Testosterone stimulates red blood cell production.
In some patients, testosterone therapy can cause a clinically significant increase in hematocrit.
This is one reason complete blood counts should be monitored during treatment.
Hormone optimization should never mean ignoring safety parameters in pursuit of a particular testosterone level.
Testosterone and Prostate Health
Prostate health should be considered according to a man’s age, medical history and individual risk.
PSA testing may be appropriate before and during treatment in selected patients.
Changes in PSA do not automatically indicate prostate cancer, but significant or unexpected changes may require additional evaluation.
Testosterone and Cardiovascular Health
Our understanding of testosterone and cardiovascular health has evolved.
Large contemporary clinical trials have provided reassurance regarding certain major cardiovascular outcomes in appropriately selected men, but testosterone therapy should not be characterized as either universally dangerous or universally protective.
Testosterone products can increase blood pressure, and individual cardiovascular risk should be considered as part of treatment and monitoring.
How Is Hormone Optimization Monitored?
Monitoring is fundamental to individualized hormone treatment.
Depending on the patient and treatment, follow-up may evaluate:
- Symptoms
- Total testosterone
- Free testosterone
- Sex Hormone Binding Globulin (SHBG)
- Estradiol when indicated
- Hematocrit and hemoglobin (via CBC)
- PSA
- Sexual function
- Body composition
- Treatment-related adverse effects
- Fertility considerations
- Other laboratory findings based on the patient’s health
A patient’s response is also important.
If laboratory values change but the symptoms that prompted treatment do not improve, the clinician should reconsider whether those symptoms are actually hormone-related.
Treatment should not simply be escalated indefinitely in pursuit of symptom improvement.
How Quickly Do Symptoms Improve?
Different effects of testosterone therapy occur at different rates.
Some men notice changes in sexual desire or energy relatively early, while changes in body composition, strength and bone health generally take longer.
Response also varies considerably between individuals.
This is another reason treatment should be assessed clinically rather than judged solely by a laboratory result.
Is Testosterone Therapy Lifelong?
Not necessarily.
The answer depends on why testosterone was initiated, whether the underlying condition is reversible, the patient’s response, adverse effects and changing treatment goals.
Men should understand that exogenous testosterone suppresses natural testosterone production.
Long-term treatment decisions should therefore be periodically reassessed rather than assumed.
Frequently Asked Questions
Can I have symptoms even if my testosterone is normal?
Yes. A laboratory reference range does not determine the cause of an individual’s symptoms. A man with testosterone within the reference range may still warrant further hormonal and medical evaluation. However, symptoms alone do not establish testosterone deficiency or prove that testosterone therapy will improve them.
What testosterone level is considered low?
The American Urological Association uses a total testosterone level below approximately 300 ng/dL as a reasonable cutoff supporting the diagnosis of testosterone deficiency when compatible symptoms are present and low levels are confirmed with repeat morning testing.
Is 300 ng/dL the ideal testosterone level?
No. It is a diagnostic threshold used in clinical guidance, not an “optimal” target for every man.
Is a higher testosterone level always better?
No. The goal of hormone optimization should not be maximizing testosterone. Excessive testosterone exposure can increase adverse effects without necessarily producing additional clinical benefit.
Why check free testosterone?
Free testosterone can provide additional information in selected patients, particularly when SHBG affects the relationship between total testosterone and biologically available testosterone.
Does every man on testosterone need an estrogen blocker?
No. Estradiol has important physiologic functions in men. Treatment decisions should be individualized rather than automatically suppressing estradiol.
Will testosterone fix erectile dysfunction?
Not necessarily. Erectile dysfunction can have vascular, neurologic, medication-related, psychological and hormonal causes. Testosterone therapy is not a universal treatment for ED.
Does testosterone affect fertility?
Yes. Exogenous testosterone can significantly suppress sperm production. Fertility goals should be discussed before treatment begins.
What is the goal of hormone optimization?
The goal is not simply to raise a laboratory number. The objective is to evaluate the patient’s symptoms and overall health, identify potentially treatable hormonal abnormalities, individualize treatment when appropriate, measure clinical response and maintain appropriate safety monitoring.
A Different Way to Think About Men’s Hormone Health
A laboratory report is an important tool, but it does not replace clinical medicine.
Two men can have the same testosterone level and feel completely different.
Likewise, two men with similar symptoms may have entirely different underlying causes.
For this reason, men’s hormone care should not begin and end with the question:
“Is my testosterone normal?”
A better question is:
“Why am I experiencing these symptoms, and what does my complete clinical picture tell us?”
